How many units of Tresiba is safe?
For Tresiba® FlexTouch® U-200, the minimum dose is 2 units, and the maximum dose is 160 units in a single injection. The Tresiba® vial allows dosing in half-unit increments. This may be helpful if you or your child is on a very small dose of Tresiba® (ie, less than 5 units).

Prescribing Tresiba. When sending prescribing information, be sure to specify the right formulation of Tresiba® for your patients: The U-100 pen has a maximum dose of 80 units per injection and is dosed in 1-unit increments. The U-200 pen has a maximum dose of 160 units per injection and is dosed in 2-unit increments.
People with kidney or liver problems may have a higher risk for hypoglycemia while using Tresiba. Before using this drug, let your doctor know if you have a history of kidney or liver problems. They can recommend ways to help prevent hypoglycemia.

Is 20 units of insulin a lot?
Basal insulin is usually started at a low dose (10 to 20 units) and then increased gradually to determine the right dose for an individual. Using a combination of treatments (ie, an oral medication plus insulin) generally lowers the dose of insulin compared with taking insulin only.
Insulin degludec, which is supplied under trade name Tresiba, is an ultra long acting basal insulin which can be used by people with type 1 or type 2 diabetes. Tresiba last for up to 42 hours offering greater flexibility as to when it can be injected each day.
Adults can take once-daily Tresiba® at any time of day and can decide on a time and place that works best. However, children must take their dose at the same time every day.
However, some research suggests that Tresiba may be more effective than Lantus at preventing hypoglycemia (low blood sugar) during the night. This may be the case for people with either type of diabetes. Talk with your doctor about which long-acting insulin is right for your diabetes treatment plan.
The CHMP concluded that Tresiba is effective in controlling blood glucose levels in patients with type 1 and type 2 diabetes. Regarding its safety, the Committee concluded that Tresiba is generally safe and its side effects are comparable to those of other insulin analogues with no unexpected side effects reported.
Taking insulin, such as Tresiba, can cause you to gain weight. In studies, people with type 1 or type 2 diabetes gained weight while taking Tresiba. Weight loss is not a reported side effect of Tresiba. If you lose weight while taking Tresiba, your doctor may change your treatment plan.

Side effects include: Insulin degludec: Hypoglycemia, allergic reactions, injection site reactions, lipodystrophy, pruritus, rash, edema, weight gain, nasopharyngitis, headache, upper respiratory tract infection, sinusitis, gastroenteritis, diarrhea.
The expected values for normal fasting blood glucose concentration are between 70 mg/dL (3.9 mmol/L) and 100 mg/dL (5.6 mmol/L). When fasting blood glucose is between 100 to 125 mg/dL (5.6 to 6.9 mmol/L) changes in lifestyle and monitoring glycemia are recommended.

Should Tresiba be refrigerated?
After first use, Tresiba® can be stored at room temperature (up to 86°F [30°C]) or in the refrigerator (36°F to 46°F [2°C to 8°C]) without the needle attached for a maximum of 8 weeks (56 days).
Tresiba® releases slow and steady from the ends of its molecular chain for a full 24 hours of blood sugar control with powerful A1C reduction. For adults, if you miss or delay your Tresiba® dose, simply take it as soon as you can, then return to your normal dosing schedule.
Choose an injection site on your thighs, abdomen (but not within 10cm of your belly button) or upper arm, and wipe it with an alcohol wipe.

The recommended starting dose of TRESIBA in insulin naïve patients with type 2 diabetes mellitus is 10 units once daily.
What to do if you've taken too much insulin. Sometimes, you can accidentally take more insulin than you need, which can result in an overdose. This can be very serious, and may lead to severe hypos. The worst cases can make you feel disorientated, cause you to have seizures and could even lead to death.
Insulin glargine: Lantus, Basaglar, and Toujeo all contain insulin glargine and last for at least 24 hours. Toujeo lasts the longest (up to 36 hours). There are also several Lantus biosimilars that have been approved. These include Semglee (insulin glargine-yfgn) and Rezvoglar (insulin glargine-aglr).
Tresiba contains the active drug insulin degludec, and it's available only as a brand-name biologic drug. It doesn't come in a biosimilar version. A biosimilar medication is a drug that's similar to a brand-name biologic drug (the parent drug). Also, biosimilars tend to cost less than brand-name medications.
The duration of action of Tresiba is beyond 42 hours within the therapeutic dose range. Steady state will occur after 2–3 days of dose administration.
What's Really Different About Tresiba? What's new about Tresiba is that it is actually a long-lasting basal insulin. It stays effective for 42 hours between doses, meaning it offers patients the flexibility to take some injections every other day, if they need to.
What is diabetic belly?
But that doesn't mean abdominal weight gain should be ignored. It can be an early sign of so-called "diabetic belly," a build-up of visceral fat in your abdomen which may be a symptom of type 2 diabetes and can increase your chances of developing other serious medical conditions.
When a person takes insulin as a treatment for diabetes, they may gain weight. This is because their body begins absorbing glucose again and converting any excess into fat. If treatment does not manage diabetes well, and blood glucose levels are too high, this weight gain can occur.
Exercise seems to work off belly fat in particular because it reduces circulating levels of insulin —which would otherwise signal the body to hang on to fat—and causes the liver to use up fatty acids, especially those nearby visceral fat deposits, he says.

Insulin has a narrow therapeutic index, which means there is a fine line between a beneficial dose and a harmful one. It's possible to overdose on insulin. In a review of insulin overdoses, excess dosages were reported as being anywhere from 26 to 4,800 units .
Insulin isn't delivered consistently when you inject into an area of fatty lumps, which means that insulin can take longer to be absorbed leading to higher blood glucose levels, or it can be absorbed too quickly, leading to low blood glucose levels. To prevent this, make sure you rotate your injection sites.
Official answer. Tresiba that has been opened may be kept at room temperature (59 to 77 degrees Fahrenheit [15-25 degrees Celcius]) for up to 56 days (8 weeks). This is much longer than most other insulins that can only be left unrefrigerated for up to 28 days.
If you stop using Tresiba®
Do not stop using your insulin without talking to your doctor. If you stop using your insulin, this could lead to a very high blood sugar level and ketoacidosis (a condition with too much acid in the blood), see advice in section 4 'Too high blood sugar'.
Check the average cost of Tresiba® for different insurance scenarios. Eligible patients with private or commercial insurance may pay as little as $35 per 30-day supply (maximum savings up to $65 per 30-day supply) or no more than $99 for up to 24 months.
Drinking alcohol while taking this medication increases the risk for a dangerous decrease in your blood sugar (hypoglycemia). Contact your care team if you experience tremors or shaking, anxiety, sweating, cold or clammy skin, confusion, dizziness, or a rapid heartbeat.
Why should you take insulin at bedtime?
Adding (clear) insulin before meals (topping up your own insulin) stops the rise in glucose levels after food. The bedtime (cloudy) insulin tops up your insulin and works with your own body's glucose, which is naturally released during the night, stopping the morning glucose levels being above the recommended limits.
Can you take Tresiba (insulin degludec) and metformin together? Tresiba (insulin degludec) can be taken with other oral diabetes medications, like metformin, if your blood sugar level needs better control. Talk to your provider about your diabetes treatment and what medications may be right for you.
In a large safety study vs Lantus® in adults with type 2 diabetes and heart disease, Tresiba® did not increase their risk of major cardiovascular events, such as heart attack, stroke, or death. Compared to Lantus®, Tresiba® also had a lower rate of severe low blood sugar, a possible serious side effect of insulin.

Tresiba may cause fluid retention. In someone with heart failure, fluid retention can worsen their condition. And if you have heart failure and take thiazolidinediones such as pioglitazone (Actos), using Tresiba may greatly increase the risk of worsened heart failure.

Use of long-acting insulin at bedtime provides excellent control of the fasting plasma glucose level.
In patients with type 2 diabetes, marked obesity, and insulin resistance, total daily insulin doses of 200 to 300 units are often required. In this setting, management for most patients includes a total of 1.0 to 2.0 units of insulin per kilogram per day; thus, in very obese patients, a larger total dose is required.
Lantus is a long-acting insulin that lasts 24 to 36 hours depending on its concentration. Tresiba is approved for use in adults and children older than one year. Lantus is approved in adults and children older than six years. Tresiba is slightly more expensive than Lantus.
